Adult 65+ years old $120/month

*2 adults, 2 children ($20/additional child)

There is a one time non-refundable $100 registration fee per household.

If you decide to cancel, it's easy, you just let us know. If you cancel then decide to rejoin, you will be asked to pay the registration fee again.

How it works

Patients are billed on a monthly basis. There is a one-time registration fee of $100 per household.

There is no long term contract, you can cancel at any time if you feel the practice is not the right fit for you. There is no cost for an introductory meeting to determine if Harmony Health is right for you.

There are NO co-pays for visits. Special pricing has been contracted for lab tests and with procedures there may be a supplies/medication cost associated. This will pass through to you at greatly reduced rates. You will be notified ahead of time if you will be responsible for any additional costs.
